Travel Vaccination Service
Hampton Wick Surgery is a registered travel clinic and Yellow Fever Centre. We provide a comprehensive travel vaccination service.
If you would like to use our travel vaccinations service, please select the link below and complete the Travel Vaccination request form. You can either complete it and email it to the practice (firstname.lastname@example.org) or you can print it off, complete and return to the practice.
NB: Please request your travel vaccinations at least 6 WEEKS before travelling, preferably as soon as you have booked your travel arrangements.
Travel Vaccination Form
Our staff will check the world health organisation current recommendations to find out what vaccinations may be needed to protect you during your trip. We will contact you within 7 working days to book an appointment, and to give you details of the recommended vaccines and the associated cost. Please make sure you provide full information of all travel destinations.
Please Note: Some vaccinations must be given 4 weeks prior to travel in order for them to be effective so we ask that you complete the travel request form at least 6 weeks prior to your travels. This is so we can order and administer the vaccines in time to ensure you are protected.
Some travel vaccinations are not provided as part of NHS services and you will be required to pay directly for the cost for some vaccines. Read the Practice Price List.
