Hampton Wick Surgery has a vibrant Patient Participation Group (PPG). The PPG meet on a monthly basis and produce our seasonal Practice Newsletter. By working in partnership with us, the PPG help to make services more user-friendly and improve patient experience. By identifying issues that are important to patients and by responding to any new initiatives, we hope we can improve and influence the way in which services are planned and delivered, and increase patients' confidence in the healthcare provided.

NHS Health Check
The NHS Health Check is for adults in England between the ages of 40 and 74. Everyone is at risk of developing heart disease, stroke, type 2 diabetes or kidney disease. But these diseases can often be prevented, and NHS Health Check can help you by assessing your risk and giving you personalised advice on how to reduce it, you can book an appointment directly with the nurse for the initial blood tests. INR testing for patients on Warfarin
Anticoagulant testing is undertaken by the HCA during normal nursing appointments. The service is availble for patients who are stable on Warfarin medication. Any patients whose INR proves difficult to control will be referred to the anticoagulation clinic at a local hospital
Diabetic clinics are run once a week—usually on a Wednesday. Please call the surgery to book in for your annual check.
If you are asthmatic and using inhalers you are advised to attend an asthma clinic at least annually. Please call the practice to book an appointment. The GP/Nurse will review your medication and give you advice on how to best manage your asthma.
Advice for patients should the GP request that you have a spirometry test:
The practice runs a stop smoking service with the Health Care Assistant. If you are interested please call the surgery to book an appointment to discuss. We can offer various stop smoking aids and give advice and guidance on how you can start taking the steps to give up.
Please leave 5 working days before trying to obtain your test results, then call the surgery after 11am on 0208 977 2638.
We are only able to give results to the patient which they relate to. To maintain patient confidentiality, prior written consent is required if another person is going to obtain results for one of our patients.
You do not need to see the GP when you first become pregnant. Instead you can self-refer directly to Kingston Hospital Maternity Services. Please complete their web form, available here.
If you do wish to book an appointment with the GP, please do so at around 6-8 weeks of conceiving. We do not perform pregnancy tests in the surgery so you will need to obtain an over-the-counter test before coming to see your GP.

After having your baby, we would like you and baby to book a 30 minute post-natal appointment with a doctor to coincide with 8 weeks after the birth date. During this visit, you will have the opportunity to discuss any issues you might be experiencing and to give your baby their first set of immunisations in order to protect them from disease.
It is of the utmost importance that children are kept up to date with the recommended vaccinations to protect them and other against disease.
If your child has not received all of the recommended immunisations, please call the surgery to book an appointment with our Practice Nurse or Health Visitor (based at Teddington Clinic).

Hampton Wick Surgery is a registered travel clinic and Yellow Fever Centre. We provide a comprehensive travel vaccination service.
If you would like to use our travel vaccinations service, please select the link below and complete the Travel Vaccination request form. You can either complete it and email it to the practice (email@example.com) or you can print it off, complete and return to the practice.

Our staff will check the world health organisation current recommendations to find out what vaccinations may be needed to protect you during your trip. We will contact you within 7 working days to book an appointment, and to give you details of the recommended vaccines and the associated cost. Please make sure you provide full information of all travel destinations.
Please Note: Some vaccinations must be given 4 weeks prior to travel in order for them to be effective so we ask that you complete the travel request form at least 6 weeks prior to your travels. This is so we can order and administer the vaccines in time to ensure you are protected.
Some travel vaccinations are not provided as part of NHS services and you will be required to pay directly for the cost for some vaccines. Read the Practice Price List.

The practice is able to provide private services for medical assessments and medical reports, as well as a 'To Whom it May Concern' letter service for patients, their insurance companies and employers. There is a charge for all of these services as they do not fall within NHS treatment. If you require this service, please see 'Practice Price List' on the bottom of this page or contact reception for more information.
